Patient Information

We want you to feel informed and comfortable at all stages of your appointment with Interboro Surgical Associates. Below, we have provided information about what to prepare before your appointment with us, what to expect during the appointment itself, and how to care for yourself after your procedure. We have also included information about directions and health insurance.

Before Your Appointment

ISA patients are referred by Primary Care Physicians (PCP) in the tri-state area. Prior to your appointment, please let us know your PCP’s name and contact information, so that we may obtain the necessary charts, examination information, and medical authorization.

To streamline your visit to Interboro Surgical Associates, please complete the Patient Registration Packet prior to your arrival and read the Notice of Privacy Practices. If you are a varicose vein patient receiving sclerotherapy, we request that you bring both the Patient Registration Packet and a signed copy of the Sclerotherapy Consent Form, also provided below, as well as read the Notice of Privacy Practices. In addition, we suggest that you prepare a list of all medications that you are presently taking, along with a list of any questions or concerns you may have. Bring these lists with you to your appointment.

During Your Appointment

In order to ensure that your visit is comfortably managed and to minimize your wait time, we ask that you arrive 15 minutes early. So that we can better assist you, please bring your new patient forms (or established patient follow-up forms), insurance card, referral, driver's license, co-pay, a list of your current medications, and any blood work or x-rays that you might have had. We also ask that you give us 24 hours' notice if you need to make any changes or cancellations to your appointment.

At your initial meeting, the surgeon will examine you and ask about your condition. He or she will speak with you about possible treatment options, including all of their risks and benefits, and he or she will answer any questions or concerns you may have. All medical decisions will be discussed with your PCP prior to the scheduling of any surgery or procedure.

As noted above under the Health Insurance section, any patient insurance responsibilities, or co-pays, must be paid upon arrival to your scheduled visit. This policy is enforced in order to avoid any disservices and inconveniences in insurance coverage to our valued patients.

The surgeons at ISA offer a full array of the latest minimally invasive surgery options. These range from non-invasive vascular studies performed in one of our offices to more advanced procedures at Lutheran Medical Center or New York Methodist Hospital in Brooklyn and robotic surgery at Lenox Hill Hospital in Manhattan.

Health Insurance

We accept various insurance carriers depending on the physician you will be seeing. Please call the office to speak with one of our knowledgeable staff members about your individual coverage.
